The use of these terms in titles and abstracts could indicate a greater understanding of the importance of pragmatism, but it isn't clear if this is evident in the contents of the articles. Conclusions As the value of real-world evidence becomes increasingly widespread the pragmatic trial has gained traction in research. They are clinical trials that are randomized that compare real-world care alternatives instead of experimental treatments under development, they involve patients that are more similar to the ones who are treated in routine medical care, they utilize comparators which exist in routine practice (e.g., existing drugs), and they depend on the self-reporting of participants about outcomes. This approach can overcome the limitations of observational research, for example, the biases that come with the reliance on volunteers as well as the insufficient availability and the coding differences in national registry. Pragmatic trials also have advantages, including the ability to draw on existing data sources, and a greater probability of detecting meaningful differences than traditional trials. However, they may still have limitations that undermine their validity and generalizability. The participation rates in certain trials could be lower than anticipated because of the healthy-volunteering effect, financial incentives or competition from other research studies. The necessity to recruit people quickly restricts the sample size and the impact of many practical trials. Practical trials aren't always equipped with controls to ensure that the observed differences aren't caused by biases that occur during the trial. The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and were published until 2022. They assessed pragmatism using the PRECIS-2 tool that includes the domains eligibility criteria and recruitment criteria, as well as flexibility in adherence to interventions, and follow-up. They found that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or higher) in at least one of these domains. Studies with high pragmatism scores are likely to have more criteria for eligibility than traditional RCTs. They also have populations from many different hospitals. The authors claim that these traits can make pragmatic trials more meaningful and relevant to everyday clinical practice, however they do not guarantee that a trial conducted in a pragmatic manner is completely free of bias. The pragmatism is not a definite characteristic the test that doesn't have all the characteristics of an explanation study can still produce valuable and valid results.
